MYSQL PHP Contract Project for Record Label

We need someone with PHP and MySQL skills to help with the backend of a music-related web promotion. This project needs to get done this week, as the promotion launches on Dec. 22, so we're looking for something as soon as possible. Our budget for this project is $500 and there will be other similar entertainment-related projects in 2009.
The task list:
1. Create a web page that will collect data from the user (name, email, gender, zip code, birthday). This page does not need to be skinned with the client's artwork.
2. Create a basic landing page that will tell the user to expect an email with instructions to download the mp3. This page does need to be skinned with the client's artwork, which we will provide.
3. Set up the back-end to collect and compile the user email address and other data, and send each user a unique user code and link to download (file hosted on client site).
4. The client would like to launch this promotion on Mon. Dec. 22, so all assets need to be in place before then.
